accessible equipment? |
| A. Yes,
please be sure to let the person making your reservation know of any
special needs you have. |

Q. Does USTA provide aides for
handicapped clients? |
| A. No,
if you need assistance, you are permitted to take a family member or
friend with you at no extra fee. A Doctor's certification is
required to document the need for a free escort. USTA drivers
will provide door-to-door service for those who require assistance.
USTA drivers are not permitted to enter consumer's homes. |

Q. How much does it cost to ride USTA? |
| A.
Snyder and Union County is divided into zones and one-way rates are
based on average miles between zones. Please call for more
pricing information. |

Q. What time will appointments be
accepted by USTA? |
| A. Trips
can be scheduled Monday through Friday (with the exception of
holidays) for appointments made no earlier than 6:30 a.m. and no
later than 4:30 p.m. Out-of-county appointments can be
scheduled no later than 3:00 p.m. Evening and weekend
group/shuttle service is available upon special request. |

Q. Are there programs available to
assist in fare fees? |
| A. Yes!
Through the Commonwealth of Pennsylvania's Shared-Ride Lottery
Program, transportation is available to senior citizens (age 65+) at
an 85% discount. Senior citizens may schedule medical, bank,
shopping, and visitation trips and are required to pay 15% of the
cost of the trip. USTA administers a grant from the
Pennsylvania Department of Public Welfare to provide free
transportation to clients with an ACCESS card. The program
will pay for trips taken to providers who accept the ACCESS card as
payment for service. Mileage reimbursement is also available
to eligible medical appointments. Other local agencies also
assist with transportation cost for certain trips. |

center for Snyder and Union Counties. |

Q. How can I make reservations for
a trip? |
| A.
Phone USTA at 522-1390, 743-0029, or toll free at 1 (877) 877-9021
between the hours of 8:00 a.m. and 4:00 p.m. The USTA office
is closed weekends and holidays. |

Q. When is the deadline for trip
reservations? |
| A.
10:00 a.m. the business day prior to your trip. Saturdays,
Sundays and holidays are not considered business days. USTA
appreciates 2 business days notice when possible. |

Q. How can I enroll in the
programs available? |
| A.
Please call USTA to register. You will be asked personal data,
including emergency contacts and special needs. Senior
citizens must be verified as age 65 prior to USTA providing
transportation through the Shared-Ride Program. If you will be
using an ACCESS card, you will be asked to provide USTA with your
recipient and card issue numbers as well as sign an application
prior to USTA providing your first trip. A separate
application and eligibility form must be completed for the
PwD
program. |
